What to Bring to Your First Appointment
To help us get started, please bring:
• Completed forms (if filled out in advance)
• A valid ID
• Your insurance information (if applicable)
• A list of any medications you are currently taking

Insurance & Coverage
We accept most insurance plans and offer direct billing to many providers for added convenience.
We also accept the Canadian Dental Care Plan (CDCP) for eligible patients.
